AgentSmith

Medical and research tools

Your agent can search PubMed, find recruiting trials, look up how a drug works and what's on its label, check ICD-10 codes and pull structured data from a de-identified note. Every answer comes with its source, so you can check it in seconds. You ask in the same chat you already use.

Before anything else: no patient identifiers

Your agent runs on an AI model hosted outside Singapore, so whatever you send leaves the country. Send the clinical picture, never the person. Your agent is set to stop and ask you to remove identifiers if it spots any.

Mdm Tan Siew Lan, S1234567D, DOB 12/03/1962, bed 14 ward 7B, T2DM, eGFR 38, on metformin 1 g BD

Name, NRIC, date of birth and location together identify her, and under PDPA that's personal data leaving Singapore.

✓ Send this instead
64F, T2DM, eGFR 38, on metformin 1 g BD. Is an SGLT2 inhibitor reasonable, and what dose adjustments apply?

Age, sex and the clinical facts are all the agent needs to find the evidence and the label.

The example patient above is made up. The same rule applies to photos of notes, discharge summaries and lab reports: crop or black out the name and NRIC first.

How it works

You send one question. Your agent picks the right source, reads what comes back and replies with a short answer and its citations.

A quick question gets a 2 to 6 line answer first, then the detail and the citations underneath.

What you can ask for

Each card is one kind of job, with lines you can send as they are. The tags show where the answer comes from.

Evidence and literature

Searches PubMed, reads the abstracts, says how strong and how recent the evidence is, and lists the PMIDs. It can fetch full text when the paper is open access.

  • Latest evidence on SGLT2 inhibitors in HFpEF, with PMIDs
  • Summarise this paper's methods and limitations
PubMed

Clinical trials

Finds trials by condition, drug, sponsor or eligibility, filtered to Singapore or the region when you're thinking about a referral.

  • Any recruiting trials in Singapore for resistant hypertension?
  • What's the primary endpoint of NCT05714085?
ClinicalTrials.gov

How a drug works

Mechanism of action, molecular target and class from ChEMBL, plus the indications and dosing sections of the label.

  • Mechanism and target of finerenone
  • What does the label say about apixaban in renal impairment?
ChEMBLFDA label

Interactions and side effects

The label's interactions and contraindications sections, the most-reported adverse events in the FDA's reporting system, and PubMed case reports.

  • Key interactions between clarithromycin and my patient's statin
  • Most reported adverse events for empagliflozin
FDA labelFAERSPubMed

ICD-10 codes

Finds and checks diagnosis codes, and shows the hierarchy under a category so you can pick the most specific one.

  • ICD-10 code for type 2 diabetes with CKD stage 3
  • Show me the codes under I50
ICD-10-CM (US)

Pull data from notes

From a de-identified note or a batch of them, it extracts the fields you name (meds, labs, diagnoses, dates) and quotes the line each value came from.

  • From these notes, extract eGFR, HbA1c and current diabetes meds
  • Abstract this discharge summary into a table
TableQuotes per value

Study design

Drafts a study protocol section by section, finds similar trials to learn from, and runs the sample-size calculation with the assumptions written out.

  • Sample size for a two-arm trial: 30% vs 20% event rate, 80% power
  • Help me draft a protocol for a pilot study on…
DocumentCalculation

Genetics and drug targets

What's known about a gene, which diseases it's linked to, and which drugs act on it, from the Open Targets platform.

  • What's the evidence linking PCSK9 to coronary disease?
  • Known drug targets for IgA nephropathy
Open Targets

Read your files

Send a PDF, a Word document or a spreadsheet (a guideline, a paper, a journal-club article) and it summarises or answers questions about it.

  • Here's the new guideline PDF. What changed from the last version?
  • Make me five journal-club questions from this paper

ICD-10 Coding

Code for T2DM with diabetic CKD
E11.22 Type 2 diabetes mellitus with diabetic chronic kidney disease.
Add a second code for the CKD stage (N18.x). US ICD-10-CM. Your hospital's ICD-10-AM coding may differ.

Even more with browser access

On their own, the medical tools only reach open sources. Once you connect browser access, your agent can also work inside your own Chrome or Edge, on the sites you're signed in to: your point-of-care references, your drug references and the journals your library pays for. It combines those pages with PubMed and the drug databases, and keeps citing every source.

Your passwords stay in your browser. The agent only reaches it while the browser is open and connected, and you can watch it work in the tab.

Point-of-care references, checked against the evidence

It reads the topic in your UpToDate or BMJ Best Practice, then checks the key recommendations against recent PubMed trials and tells you where they differ.

  • Check UpToDate on hyponatraemia correction rates and compare with any trials since 2024
  • What does BMJ Best Practice recommend for new AF in a 70-year-old, and how strong is it?

Local drug information

Lexicomp, MIMS or your hospital formulary for local brands, dosing and renal adjustment, set side by side with the FDA label and adverse-event data.

  • Local brands and renal dosing of empagliflozin in MIMS, compared with the FDA label
  • Is rivaroxaban on our formulary, and what restrictions apply?

Full text behind a paywall

The tools find the paper on PubMed. Your library login opens the full text, so it can appraise the methods instead of only the abstract.

  • Find the main RCT for finerenone in diabetic CKD, open the full text through my library and appraise it
  • Prepare journal club for this NEJM paper: summary, critique and five questions

Guidelines and registries

Local guidance (MOH, ACE clinical guidance, college and society sites) and trial registry pages, combined with the ClinicalTrials.gov search.

  • What does the latest ACE guidance say on SGLT2 inhibitors in CKD?
  • For NCT02814942, open the study site page and list the contact and inclusion criteria

Switching it on

  1. Create a separate Chrome or Edge profile for your agent, and install the free BrowserSkill extension in that profile.
  2. In your Command Center, open Chat, then Browser access, and press Get pairing link.
  3. Paste the link into the extension, then sign in to your references and library in that profile yourself.

Keep patient records out of reach

What it is and isn't

What it does

  • Finds and summarises evidence, with a PMID, NCT number or label section for each claim
  • Says how strong the evidence is and how recent
  • Flags where Singapore practice (MOH guidance, HSA labels, ICD-10-AM) can differ from US sources
  • Explains the wording of a radiology or pathology report you've de-identified

What it doesn't do

  • It doesn't make clinical decisions. You do, with your own judgement and local guidance
  • It doesn't read scans or ECGs as a diagnostic report
  • Drug labels are US FDA labels, so check doses against your formulary before prescribing
  • It isn't a complete interaction checker, so it cites the label and literature it used

Getting the best out of it

  1. Ask like a clinical question.Population, intervention, comparison and outcome gets a tighter search than a single drug name.

  2. Say how deep to go."Two lines" for a quick check between patients, or "full review with a table" when you have time.

  3. Ask for the paper."Get the full text of PMID 42518892" works for open-access papers in PubMed Central.

  4. Ask for a file."As a table" or "as a Word document" gets you a file in the chat you can keep or share.

If something goes wrong

It stopped and asked me to remove details
It found something that looks like an identifier. Send the message again without the name, NRIC, date of birth or location.
No results came back
Try broader terms or a synonym. It falls back to a web search if a source is down, and says so when it does.
The dose or code doesn't match what I use
Its labels and codes are US versions. Tell it your local value and it will use yours.
